Out of respect for the privacy of the adult and families we serve, adults who may be eligible to receive a bucket list wish can be referred by one of three sources:

• Self Application, you must be between the ages of 19 and 65
• Medical professionals treating the adult – such as doctors, nurses, social workers or adult-life specialists
• A family member or a friend of the potential bucket list wish adult

The All About HOPE, Celebration of Life Program respects the privacy of the adults and families it serves. All medical information is considered confidential and is not discussed with outside parties unless it is required for the wish and the adult has given their consent.
